USE OF IMAGE AGREEMENT:

By completing this form, this membership applicant gives permission to Collaborative Opportunities for Raising Empowerment, Inc. (hereinafter, "CORE") and/or other sponsor(s) or partner(s) of CORE to use my likeness, alone or with others, by means of photograph, video or other medium for use in promotional materials or publicity, without notice to me and without a caption that identifies me. I understand my likeness may appear in internal publications, promotional audio/visual presentations, or external publicity, including, without limitation, use on the CORE website or its periodic newsletters or annual report. I agree that I shall have no right to (i) pre-approve use of my likeness as described above, (ii) claim compensation for the use of my likeness as described above, and (iii) make any claims based on the use, or non-use, of my likeness as described above, including any claims for libel, defamation or invasion of privacy. I agree that CORE and/or sponsor(s) or partner(s) of CORE, as the case may be, shall own all images taken by it or its representatives.

FRAUD POLICY:

Any membership applicant who knowingly and with intent provides fictitious, exaggerated, and/or otherwise misleading information on this form risks losing all membership benefits and associated fees as described above. Any fraudulent information, if discovered, reported, and investigated by CORE, Inc., proves to be as such, the offending party will face immediate dismissal from CORE without recourse and any future membership requests by the offending party will be denied. By completing this form, this membership applicant understands this policy and willingly submits information that is accurate to his/her knowledge at the time of completion.

MEMBERSHIP POLICY:

By completing this form, the membership applicant understands the policies set forth as such. Membership is an annual benefit to the applicant. All membership applicants are expected to honor the standards, policies, processes and mission of CORE during organizational events, sponsorships and partnerships. If circumstances warrant, your membership may be terminated at any time without prior warning. If this termination is at the request of the organization, your membership fee will not be refunded. If the termination is requested by the member, the membership fee will be refunded, less the non-refundable fee.

PRIVACY POLICY:

Collaborative Opportunities for Raising Empowerment, Inc. will not sell, share, or otherwise disclose (to the extent permitted by law) any of the Personally Identifiable Information (hereinafter "PII") it collects about you without your prior permission; provided that CORE may disclose such information to third parties who design and/or monitor the Web site, to parties who assist or cooperate in preparing mailings, to parties with whom CORE may co-sponsor programs or events (in which case CORE may share attendee lists for purposes of such program or event), and to non-profit partner organizations that may request professional assistance that match your skills or interests.

CORE collects PII such as your name, address, e-mail, and other contact information at events or via our website. CORE collects PII only to the extent that such information is provided voluntarily when you submit a feedback/inquiry form, send electronic mail to CORE or any of its officers, register for events, subscribe to newsletters, or submit any other content through our listserves or website. PII collected is stored in CORE's contact database where it may be combined with pre-existing contact information in that database. CORE takes reasonable security precautions to maintain the privacy of the data that is collected, such as limiting access to such data to appropriate officers under a system that is user ID and password protected. CORE requires similar basic safeguards against inappropriate use of PII from the third parties it deals with.
